Atypical presentation of incomplete duplex collecting system simulating cystic renal image: in a case report

Abstract

Description of the clinical case: A 24-year-old female comes to our unit with colic pain in the bilateral lumbar area and recurrent urinary tract infections. Right nephrolithiasis and Left Renal Cyst Bosniak II was diagnosed. Flexible ureteroscopy with laser lithotripsy was performed and in a second time was planned Laparoscopic Renal Cyst Unroofing, during surgery, the presence of a Duplex collecting system was observed, and it was decided to perform a left partial nephrectomy.

Relevancy: The low incidence of a double collecting system may cause the urologist to mistake a cystic lesion for an anatomic alteration, so it is important to show this variability as an option to cystic imaging.

Clinical implications: The duplex collecting system is a common anomaly, asymptomatic, in which the prevalence is 0.7-4%, with multiple presentations, which can generate complications mainly lithiasis, obstructive uropathy, and ureteral reflux, so it is important its diagnosis and timely treatment.

Conclusions: In patients with the presence of cystic image that generates focal hydronephrosis, an incomplete duplex collecting system should be considered as a differential diagnosis.
