Madigan technique robot-assisted simple prostatectomy: an option for ejaculatory preservation

Abstract

Case description: The objective of this study is to describe the robot-assisted Madigan technique and report our initial experience in three cases with benign prostatic hyperplasia who underwent simple robot-assisted prostatectomy with this technique. No major complications were reported in the included cases. An improvement in urinary symptoms and preservation of ejaculatory and erectile function were demonstrated in all patients during their follow-up.

Relevance: Simple prostatectomy and endourological procedures for the management of benign prostatic hyperplasia continue to have the disadvantage of presenting inevitable adverse effects, such as ejaculatory disorders. The Madigan technique for simple prostatectomy is an alternative that allows to preserve the prostatic urethra and bladder neck in order to avoid ejaculatory.

Clinical implications: The robot-assisted Madigan technique is a therapeutic option that should be considered in a young patient with obstructive prostatic growth who wishes to preserve ejaculatory function after surgical treatment.

Conclusions: Prospective studies are required to compare the different simple prostatectomy techniques to validate the role of this technique in the preservation of ejaculation.
