Predictors of delayed kidney graft function: a retrospective study

Abstract

Objective: To know the frequency and factors associated with delayed graft function (DGF) in a tertiary care center in northwestern Mexico.

Methods: Retrospectively, transplanted patients from 2011 to 2017 and their association to DGF were reviewed. The DGF was defined as a case that required dialysis within the first post-transplant week. For the analysis of the variables involved in DGF, logistic regression was used.

Results: Delayed graft function occurred in 11% (CI 95%, 7 to 15) of 199 transplants studied. The risk factors associated with DGF included: combined renal replacement therapy (OR: 3.50; CI 95%: 1.39 to 8.78; p=0.005), cadaveric donor (OR: 8.06; CI 95%: 1.82 to 35.53; p=0.001), cardiovascular disease (OR: 4.32; CI 95%: 0.74 to 25.12; p=0.07), and acute rejection (OR: 2.78; CI 95%: 0.98 to 7.90; p=0.04). The multivariate analysis demonstrated combined renal replacement therapy (OR: 3.34; p=0.01) and cadaveric donor (OR: 8.02; p=0.007) as independent risk factors for DGF (OR: 8.02; p=0.007).

Conclusions: The factors associated with DGF observed in the present study are related to the kidney recipient and the donor. Further prognostic studies that permit the discovery of the influence that diverse known factors have in the development of DGF and the prognosis for the transplanted organ are required.
